Client Rights & Consumer Information

M.B.S Whole Health is committed to transparency, ethical practice, and protecting your rights as a client. The information below explains how you may request your health care records, contact licensing authorities, and file a complaint if needed.


Requesting Your Health Care Records

You have the right to request access to your health care records.

To request a copy of your records from M.B.S Whole Health:

  1. Email our office at clients@mbswholehealth.com with the subject line “Records Request.”
  2. A signed Release of Information (ROI) is required before any records can be released.
  3. Once your completed ROI is received and your identity is verified, records will be provided in accordance with applicable state and federal laws.
  4. Fees for record requests, when applicable, are outlined in our intake and financial policies.

Licensure & Oversight

M.B.S Whole Health is a virtual-only practice. Services are provided only to clients who are physically located in a state where the provider is licensed at the time of service.

Filing a Complaint or Contacting a Licensing Authority

If you have a concern regarding services provided by M.B.S Whole Health, you may contact the appropriate licensing authority or submit a consumer complaint using the options below.


Texas Clients

Texas Behavioral Health Executive Council (BHEC)
BHEC regulates Licensed Professional Counselors in Texas.


Texas Office of the Attorney General – Consumer Protection Division

Consumers may also submit a complaint to the Texas Office of the Attorney General.


Michigan Clients

Michigan Department of Licensing and Regulatory Affairs (LARA)
Bureau of Professional Licensing (BPL)
BPL regulates Licensed Professional Counselors in Michigan.

Complaints regarding licensed providers in Michigan are submitted through the MiPLUS system.
