Requesting Your Health Care Records
You have the right to request access to your health care records.
To request a copy of your records from M.B.S Whole Health:
- Email our office at clients@mbswholehealth.com with the subject line “Records Request.”
- A signed Release of Information (ROI) is required before any records can be released.
- Once your completed ROI is received and your identity is verified, records will be provided in accordance with applicable state and federal laws.
- Fees for record requests, when applicable, are outlined in our intake and financial policies.

Licensure & Oversight
M.B.S Whole Health is a virtual-only practice. Services are provided only to clients who are physically located in a state where the provider is licensed at the time of service.
